Medication Management Project
Local pharmacists provide opportunities for group learning and one-on-one consults for persons over 60 to help answer questions about medications.  The group learning typically takes place at senior apartments or senior centers within the region.  One-on-one consultations typically take place at the

What's covered during the group learning?  General information about all forms of medications, such as over-the-counter, prescription and herbal remedies.  The sessions last about an hour.

What can you expect during one-on-one consultations?  The pharmacist reviews all your medications and supplements (including vitamins).  You'll be advised on the following:  drug-drug interactions, drug-condition interactions, appropriate doses, correct drug administration and usage, proper monitoring of drug therapy, therapeutic duplication, any potential cost savings available.

Senior Surf Days…Catch the Wave
During these half day events, older adults learn to "surf the web"…navigating the world wide web, specifically Social Security's internet website among others.  Computer basics, e-mailing and opportunities for learners to explore areas of interest during the class times.

Supporting Seniors Across Systems Project
Oftentimes as people age they need support across formal and informal care systems to help maintain their health, well-being and independence.

Positive health outcomes for older adults rely on medical treatment, rehabilitation services and home health care.  Add to that mix social service supports such as home delivered meals, transportation, help with outdoor chores and caregiver support, we get closer to reaching most people's support needs. 

What's missing?  The practice of supporting the spiritual needs of frail or chronically ill persons adds a significant component to their holistic care.  Frequently medical, social and spiritual care are integrated to provide a well-rounded approach to caring.

What's the purpose of the project?  To provide a framework connecting the three systems (medical, social and spiritual) linking seniors to services provided by faith-based networks, such as parish nursing to reduce health care and nursing home care costs while supporting improved quality of life.
